Billing and Insurance

We are no longer contracted with health insurance companies and do not accept and process any insurance claims. Our practice is strictly fee for service (cash pay only).

For your convenience, we accept cash, checks, debit cards, Visa and MasterCard.

All office visits are expected to be paid in full at the time of service. Please call the office at 425-697-5583 for more information about our fee schedule.

Blood tests and referrals for diagnostic testing such as mammogram, ultrasound, MRI, etc… can be submitted to insurance company for processing. We will be asking you for a copy of your insurance card at the time of the visit and will be submitting your insurance information to the lab who performs these services. We are not responsible for any balances accrued on your account for blood tests or diagnostic images. These are your responsibility.  

A copy of the super bill with proper CPT and ICD-10 codes (diagnosis codes) will be provided at the end of the visit. You can submit the bill directly to your insurance carrier at your earliest convenience. Again, we have no control over how much your insurance will reimburse you for the visit fee – please contact your insurance directly for the status of these claims. In many cases, your insurance company will send a payment directly to you to pay for some of the expenses spent at the office. Reimbursement varies greatly from various insurance carriers and it is your responsibility to find out about your benefits.
